Aims and objectives
- Provide person centred care and support together with a needs led 1:1 support plan that recognises the uniqueness of each individual.
- Provide opportunities for people supported that enable personal growth, confidence building and self-development.
- Support individuals to live independently by the development of daily living skills.
- Support individuals to achieve optimum health and well-being.
- Enable people to be integrated into their community by encouraging participation into activities.
- Adopt a multi-disciplinary approach involving all relevant professionals.
- Involves people supported in decisions making around their support, providing choice and control.
